How to file a claim

Initiating a Claim

  1. Take your vehicle to your selling dealership or a repair facility.
    If returning to selling dealer is not possible, call Axiom at 844.252.0937 and we will refer you to an authorized repair location.
  2. Obtain Authorization
    Prior to any repair being made, contact us to obtain your authorization number. If an emergency occurs which requires a repair to be made when our office is closed, contact us during normal business hours immediately following the repair.
  3. Authorize Breakdown
    You may need to authorize the repair facility to diagnose the vehicle to determine the cause of breakdown and if any parts will not be covered. You will be responsible for the cost of diagnosis if the diagnosis is not included in the covered repair. We reserve the right to inspect the vehicle before any repairs are performed.
  4. Submit Repair Order for Payment
    Once authorization is obtained and the repair is completed, all repair orders and documentation must be submitted to the administrator along with the claim authorization number within 60 days.
